COVID-19 Client Agreement

We appreciate your cooperation in keeping our Summit community as healthy as possible. By

entering our Facilities, you agree and accept the following terms and conditions for yourself and

those in your care.


a. I confirm I do not have any symptoms related to and have not been knowingly

exposed to COVID-19.

b. If the above status changes, I agree not to visit the club.


a. I agree to keep 1.5 metres between myself and other members while in the club.


a. I agree to disinfect all equipment after use, and will not enter closed off areas.

b. I agree to keep my hands washed/sanitized while at the club.


WAIVER: In light of the COVID-19 pandemic, I reaffirm the Waiver which I executed on my Member

Agreement, and assume the risk of use of the facilities at Summit.

Summit Staff will be monitoring the facilities:

1. Focusing on cleaning

2. Discouraging close proximity between members

3. Encouraging members to wipe down equipment.

If a staff member or member asks you to do one of the above, we ask that you please cooperate and

do your part. Thank you for your cooperation in making this a safe and healthy environment.
